Is American Express accepted at most businesses in DR? We've had problems at other caribbean locations trying to find someone who will accept it.

You are probably better off with Visa or MC.
$US cash is best.

Heck, sometimes in North America it's hard to find places which accept Amex (their service charge is higher than Visa or M/C to the merchant). Other than some hotels I doubt many places take it.
